“The following is the manuscript of a ‘work stopped in progress’ in 1994.  That is, it is a book that we started, but never finished.  Nevertheless, we used the manuscript in its various forms for tutorials and courses that we taught, always with the expectation that we would finish. Well, the reality is, that is not going to happen.
